Fix duplicate steps issue

We are getting duplicate steps, need to find a way around this.
The only solution I can think of at present is to use find_or_create instead of find_or_initialize

Now using find_or_create instead of find_or_initialize which seems to solve the issue.

Doing this stops us from actually importing a features stories, will need to look into this further.

I've reverted the find_or_create_by_title for our steps, this seems to work fine for importing all features, will check this with importing a single feature later

It seems that the issue lays with importing single features, when we do the import, the feature has no stories. Our steps are not duplicated but we need to address this side effect before we close this ticket.

Can't see an easy fix for this, will move it out and deal with it later.

Still trying to find a decent way of dealing with this.
The next best thing is to rewrite Salad::FeatureFile.
